Starting January 19, 2026, access to the Community Health Record (hie.ehoag.org) will no longer be available.
As an alternative to the Community Health Record, Hoag physicians and internal staff can access patient records by logging directly into Epic. For staff and users who do not have access to Epic, they can access patient records by using EpicCare Link. Access to EpicCare Link can be obtained by emailing HoagMedicalRecords@hoag.org.
